One of the tools we use to accomplish this is remote health monitoring—which research has shown to improve outcomes for patients with a wide variety of health conditions.

 

What Is Remote Monitoring?

You are probably familiar with common remote monitoring devices currently on the market like FitBit or Apple Watch. These track sleep patterns, heart rate, steps, etc. and allow you to view trends in your health data.

The remote monitoring tools we use—created by a US-based company called Qardio*—are designed specifically for use in a health care setting.

Instead of simply looking at steps and sleep patterns, they collect data such as blood pressure, weight, BMI, and, when relevant, blood glucose levels. 

With your permission, this data will be shared with your Whole Health Partners team  in real time to help you reach your health goals.
