Please read the return policy carefully before purchasing a Qardio Remote Health Monitoring Package.

 

Financial Responsibility

The device(s) / kits are owned by Qardio, Inc. The device(s) must be returned to Qardio, Inc within 30 days of the end of your participation in the Remote Monitoring Program. Please do this as a matter of urgency, as other patients may be waiting for you to return this monitor for testing. Failure to return the device(s) or irreparable damage to the device(s) caused by neglect while you are taking part in the program, may result in the below fees being charged based on the device(s) involved. By not returning the devices within the 30 day period following the end of your program, you are agreeing to purchase them at the below prices and will be billed accordingly.

Qardio Arm: $65.00
Qardio Base: $100.00

Any of the above fees incurred are the responsibility of you as the patient, NOT your insurance carrier. If charges are incurred, then Whole Health Partners will bill you directly. It is imperative that you take care of the monitor as if it’s your own device and return it promptly once you are finished with it. Please return all the contents in the RPM Kit box when you return the device(s).

Purchasing Devices

If you would like to keep your QardioArm or QardioBase, they can be purchased from Qardio at the above prices. In this case, let us know anytime during your program or within the 30 days following and Whole Health Partners will bill you directly.
